Tesamorelin is the full 44-residue sequence of human growth hormone-releasing hormone, GHRH(1-44) amide, carrying one synthetic addition: a trans-3-hexenoyl group acylated onto the alpha-amine of the N-terminal tyrosine. That six-carbon acyl chain, with a single carbon-carbon double bond in the trans configuration, turns the free N-terminus into an amide. Apart from the cap, every residue in the chain is a standard L-amino acid in its native position.

The cap is the whole point of the name. Remove it and the molecule becomes plain GHRH(1-44) amide, about 96 Da lighter; truncate the chain at residue 29 and you are in Sermorelin territory, which has neither the cap nor the last 15 residues. Tesamorelin is a nonproprietary name, so the structure with its hexenoyl group, not the label, is what identifies the material.

How Tesamorelin is analyzed

The hexenoyl group leaves several fingerprints. It adds hydrophobic character at the N-terminus, so Tesamorelin elutes later than the uncapped chain would under the same gradient. Material in which acylation failed shows up as a species about 96 Da light, and a cis double bond in place of the trans one is an isomer of identical mass that only chromatography can separate. Because the N-terminal amine is acylated, the molecule also has one fewer protonation site than uncapped GHRH, a small but real difference in its charge behavior. The only sulfur in the formula is methionine 27, whose sulfoxide appears 16 Da heavy.

Two tyrosines and one phenylalanine, with no tryptophan, give only modest 280 nm absorbance, so purity is integrated at roughly 214 nm.
